Position Information

Job Description

Position serves clients primarily in the Franklin County Health Department and as needed in other West Piedmont Health District offices. Independently provides nursing care to patients in clinics, homes, the community, and schools. Serves as charge nurse for clinics at the Franklin County office. Performs nursing assessments—may perform lab tests, take measurements, develop and implement care plans including the administration of medication and case management. Performs Nursing Home Preadmission Screenings. Serves as Tuberculosis Case Manager. Serves as case manager for clients in the Maternal Infant Care Coordination Program. Makes decisions based on program standards and protocols. Prioritizes assignments. Serves as Health Department representative on assigned committees.

Minimum Qualifications

Actions performed at this level require licensure as an RN according to the Virginia State code. Significant experience with physical, psychosocial, and environmental assessment. Knowledge of public health principles including professional standards, health promotion/disease prevention, growth and development throughout life stages, epidemiology, lifestyle/behavior modification. Knowledge of available community resources. Significant experience with case management and coordination of care. Experience with home visiting. Experience with individual and group teaching in office and community settings. Experience in performing epidemiologic investigations for communicable disease. Experience with and knowledge of Nursing Home Preadmission Screenings and DMAS policies and procedures. Experience with child and adult immunizations in clinic and community settings. Ability to communicate and document orally, in writing, and electronically.

Preferred Qualifications

Prefer BSN, MPH, or significant public health nursing experience. Prefer bilingual.

Special Requirements

May require occasional overnight travel or extended work hours to meet agency needs. Employment is contingent upon satisfactory results of a state and federal criminal history background check and the Department of Social Service’s Child Abuse and Neglect Central Registry check, U.S. HHG IG Exclusion List check, employment reference check, and E-verify. The selected candidate must complete a Statement of of Personal Economic Interest as a condition of employment, if applicable (Va. Code § 2.2-3114). Other financial, credit, driving, or other background checks prior to employment may be required for certain positions.
